Below are the AFC Vacations Reservation forms which are required to be completed and returned to our office. This information is required by TSA and must be completed before travel.

If you are traveling Domestically please print, complete and return the Domestic Reservation Form

If you are traveling Internationally please print, complete and return the International Reservation Form

If you are travelling on a Cruise, please print, complete and return the Cruise Reservation Form

If your cruise is on Holland America Cruise lines please be sure to also fill out & return the Holland America Immigration form. This form is required to release travel documents.
